If you click below, it will take you to the Application, BUT to use it you must first “Download” it and then save the downloaded file to your device or to a USB drive.

If you’ve done this successfully you will be able to type answers to questions right into the form and then save them. If you need assistance doing this Calvert Librarians are ready to help you.

When you save the Application, please modify the file name by adding your name to the end of it

(for example, “C4 Client Application 2023 Mary Green.doc”).

There is a lot of important information on the first page of the Application. Indeed, we recommend that you read all the way through the Application before beginning.

*If you are choosing to email the completed application to C4, please make sure to email the application through a secure or encrypted email setting. This will help ensure your data remains protected. You may omit a social security number and similar identifiers and if you do so, you will be contacted later as needed. You may, of course mail your application to the address indicated on the application and it will be considered received by C4 by the April 10th due date if postmarked no later than that date.*
